Physician coverage for telehealth platforms that need more capacity
Caracola Medical is a physician-led, white-label telehealth medical group. We act as a telehealth provider network for existing platforms and marketplaces, adding overflow physician capacity across all 50 states + DC or serving as the clinical partner your customers need. Founded by J. Chance Miller, MD and Stephanie Miller.

Who is this physician coverage for?
It is for telehealth platforms and marketplaces that already have demand and need licensed physicians to meet it. Some need overflow capacity on busy days. Some need reach in states their current clinicians don't cover. Others want a clinical partner they can offer to their own customers.
If your product routes patients to clinicians, and the clinician side is where things slow down, this page is for you.
What goes wrong when a platform runs short on clinicians?
Work piles up in places nobody is watching. Our founder has spent years on the clinician side of platforms like yours, and these are the things operators tell us first.
- "Orders sit in the queue with no clinician assigned, and support doesn't hear about it until the patient complains."
- "We have demand in states where we have nobody licensed, so we turn it away or route it badly."
- "Escalations live in email threads. By the time the right physician sees one, three people have already replied."
None of these are software bugs. They are coverage and ownership problems. They get fixed when a physician group takes clear responsibility for the clinical queue.
How does Caracola add physician coverage to a telehealth platform?
We supply licensed physicians, clinical protocols and day-to-day clinical operations, and we work inside the platform you already run.
Overflow capacity
Physicians who pick up the visits and orders your current clinicians can't reach, so the queue keeps moving.
50 states + DC
Physicians licensed in all 50 states and DC, so you stop turning away demand.
Clear escalation paths
One documented route for clinical escalations, with an owner, instead of scattered email threads.
A clinical partner for your customers
A white-label physician group you can offer to the brands and clinics on your platform.
English and Spanish coverage
Bilingual physicians for Spanish-speaking patients, without a separate workflow.
Documentation standards
HIPAA-compliant workflows and charting standards described on our compliance page.
How does it work?
Every partnership follows the same three steps. The pace depends on your states, your scope and how much is already in place.
Discovery call
We learn how orders and visits move through your system, where they stall, and which states and service lines matter most.
Program design
We agree on protocols, routing rules, escalation paths and documentation, and start licensing and credentialing where needed.
Launch and grow
Our physicians start taking work in your queue. We review volume and gaps with you and add states or service lines as you grow.
Already have a platform? No tech yet?
Already have a platform? Good. We work inside it. We have run clinical operations inside most of the major telehealth platforms and EHRs, so your intake, routing and charting stay where they are.
No tech yet? That usually applies to your customers, not you. No tech yet? We'll point you to the right platform based on what we've actually run. We don't sell software, so the advice isn't tied to a product. Our guide on how to choose a telehealth physician network is a good place to start.
Frequently asked questions
Can a telehealth provider network plug into an existing platform without changing our workflow?
Yes. We work inside your platform and EHR. We learn your routing and documentation rules and follow them, then suggest changes only where they fix a real problem.
How do you add physician coverage to a telehealth platform in new states?
We provide credentialing and state licensing support for the states you serve, and we expand coverage as your demand grows. We don't promise specific dates, because licensing timelines depend on each state.
Who makes the clinical decisions?
Licensed clinicians do, independently. Patients interact with your brand, and required clinician disclosures are given as state law requires.
Do you offer Spanish-speaking physicians?
Yes. Bilingual English and Spanish physician coverage is available. Our founder is fluent in Spanish and still sees patients each week.
