Medical weight loss

A physician network for your GLP-1 program, under your brand

Caracola Medical is a physician-led, white-label telehealth medical group. We supply GLP-1 prescribing physicians for brands, along with the clinical protocols and day-to-day clinical operations a medical weight loss program needs across multiple states. Founded by J. Chance Miller, MD and Stephanie Miller.

The clinical layer

What does a compliant GLP-1 program need clinically?

A compliant GLP-1 program needs a real clinical evaluation behind every prescription. That means a complete intake, contraindication screening, labs when the protocol calls for them, informed consent, clear documentation and a follow-up cadence set by protocol. The prescription is the last step, not the first.

Our founder has worked the clinician side of programs where an order arrived with no intake attached, or with a questionnaire that skipped the history that mattered most. That is where the risk sits, for patients and for the brand. We build the clinical layer so a physician always has what they need to make a decision.

Complete intake

Medical history, current medications, prior treatment and goals, captured in a structured form the reviewing physician can actually use.

Contraindication screening

Screening questions written into the protocol, with clear stop points that route a patient to a clinician instead of a checkout page.

Labs when indicated

The protocol defines when labs are needed. The clinician can always request more based on what the intake shows.

Informed consent

Plain-language consent that covers risks, alternatives and what the program does and does not include.

Documentation

Every decision charted in your system, including why a patient was approved, deferred or referred elsewhere.

Follow-up cadence

Check-ins scheduled by protocol, so continued treatment is always tied to a recent clinical review.

Clinical decisions are made independently by licensed clinicians. A completed intake is a request for evaluation. Whether to treat is a clinical decision.

Sync vs async visits: when is each used?

It depends on the rules in each state your patients are in and on how your program is designed. Some situations call for a live audio or video visit, and others allow an asynchronous review of a complete intake. We map this state by state before launch, so your workflow matches the rules where you operate.

How visit type gets decided
FactorWhat we look at
State rulesWhat each state in your footprint requires before prescribing, and for follow-up care.
Program designWhether your model is built around live visits, async review, or a mix of both.
Intake qualityAn incomplete or unclear intake is a reason to talk to the patient, not to guess.
Clinical findingsAnything in the history that warrants a conversation moves the patient to a live visit.
LanguageWhether the patient is better served by a Spanish-speaking physician.

One practical rule holds everywhere. A clinician can always move a patient from async to a live visit, and your platform should make that a single step, not a support ticket.

How are titration, follow-up and side effects handled?

Through protocol-driven physician review. Each follow-up is reviewed by a licensed clinician against your program's protocol, with a defined escalation path for anything outside it and clear routing for patient questions. Dosing decisions live in the protocol and in the clinician's judgment, not in marketing copy or a support script.

  • Scheduled reviews. Continued treatment is tied to a follow-up the protocol defines, and the clinician decides whether changes are appropriate.
  • Side effect reports. Reviewed by a clinician, documented in the chart, and answered with clinical judgment rather than a canned reply.
  • Escalation path. A written path for messages that need a physician the same day, a referral to in-person care, or emergency guidance.
  • Support routing. Billing and shipping questions go to your team. Clinical questions come to ours. Both sides know which is which.
  • One escalation channel. Urgent issues surface in one agreed place, with a named owner, not across scattered email threads.

Most of the problems we have been asked to clean up were routing problems. A side effect message sat in a shipping queue. An escalation lived in three inboxes and nobody owned it. We agree on who owns what before the first patient is seen.

How we staff your program: medical weight loss physician coverage

We staff for coverage, not headcount. Coverage planning starts with your states, your visit model and your expected demand pattern. Then we assign licensed physicians who learn your protocol and stay with your program.

Coverage planning

Physician hours matched to when your patients actually submit intakes and send messages, including evenings and weekends where your program needs them.

Bilingual EN/ES

English and Spanish physician coverage, so Spanish-speaking patients can have their visit in their own language.

Consistent physicians

A stable group of physicians who know your protocol, your platform and your escalation path. Patients see familiar names.

Licensing support

Credentialing and state licensing support as your program moves into new states.

If your structure also calls for a medical director or supervising physician, see our medical director and physician oversight services. For how the white-label model works in general, read how white-label telehealth works with Caracola.

Working inside your platform and pharmacy partner

We work inside the platform and pharmacy relationships you already have. Our physicians review, document and prescribe in your system, and prescriptions route to the pharmacy partner your program uses, subject to state rules and the clinician's judgment.

We do not sell software, so we have no reason to push you toward a particular platform. If you have not chosen one yet, we can help you weigh options on what matters to clinicians: intake logic, async review, prescription routing, lab ordering and how escalations reach a physician. Platform teams can read more on our page for digital health platforms.

Compounded medications are not FDA-approved. If your program works with a compounding pharmacy, patient materials and consent should say so plainly, and every prescribing decision stays with the treating clinician.

How to launch a GLP-1 program with our physician network

Launch follows a clear sequence: discovery, protocol, licensing and coverage, platform setup, then a supervised go-live. Pace depends on your states and your platform, so we build the plan with you rather than promise a date.

Discovery call

We learn your program, your states, your platform and your pharmacy setup, and tell you plainly where we see gaps.

Protocol

We write or review the clinical protocol: eligibility, screening, labs, consent, follow-up and escalation.

Licensing and coverage

We confirm licensure for the states you serve and build a coverage plan around your visit model.

Platform and pharmacy setup

We configure clinician workflows in your platform and put the HIPAA agreements the relationship requires in place before launch.

Supervised go-live

Test cases first, then live patients, with close review of the early charts and routing.

Ongoing review

Regular check-ins on chart quality, escalations and protocol updates as rules and your program change.

FAQ

Frequently asked questions

What is a physician network for a GLP-1 program?

It is the group of licensed physicians who evaluate patients, prescribe when appropriate and handle follow-up for your weight loss program. With Caracola, those physicians work under your brand, inside your platform, following a protocol we agree on together.

Do your GLP-1 prescribing physicians work under our brand?

Yes. Patients interact with your brand. Our physicians provide the clinical care, and the clinician disclosures each state requires are given as state law requires.

Can your weight loss telehealth providers work inside our existing platform?

Yes. We work inside the platform or EHR you already use. If you have not chosen one, we can help you compare options, and because we do not sell software our advice is not tied to a product.

Is every patient who completes an intake prescribed medication?

No. Each patient is evaluated by a licensed clinician who makes an independent decision. Some patients are approved, some need more information or labs, and some are better served by another option or in-person care.

Do you offer medical weight loss physician coverage in Spanish?

Yes. English and Spanish physician coverage is available, and our founder is fluent in Spanish.

Do you work with compounding pharmacies?

We work with the pharmacy partner your program uses, subject to state rules and clinical judgment. Compounded medications are not FDA-approved, and we expect patient materials and consent to say so clearly.

Who writes the clinical protocol for our GLP-1 program?

We write it with you or review the one you have. Your business goals shape the program design, and clinical content is set by physicians. Once live, physicians apply the protocol with independent judgment.

Which states can your physician network cover?

Our physicians are licensed in all 50 states and Washington, DC. During discovery we confirm licensure for the states you serve and plan licensing support for states you want to add. More answers are on our partner FAQ.
